Responsibilities:
Provide educational in-services and other educational opportunities to the community and providers in assigned territory.
Work cross-functionally, with Admissions and Marketing Departments, to develop and carry out a marketing plan.
Manage the marketing activities for an assigned coverage area, daily.
Meet with appropriate parties to establish a positive referral relationship and to educate on referral appropriateness.
Develop opportunities to conduct in-service presentations for physician offices, hospitals, nursing homes, assisted living, Community Based Residential Facilities (CBRF) and other healthcare providers.
Collaborate with Admission Department and clinical staff to manage intake of patient and families.
Coordinate utilization review with providers.
Meet or exceed Key Performance Indicators (KPIs) including; make 10 face to face contacts per day with referral sources, this could include multiple contacts within one facility, but must be face-to-face contacts with staff who have an impact on referrals - dropping off literature or telephone calls do not count towards the daily goal.
Visit at least 6 facilities per day, develop business month over month growth, meet quarterly admission goals, responsible for maining accurate daily, weekly, monthly, and quarterly admissions numbers for their territory and quarterly plan, maining 100% daily documentation in Customer Relationship Management System (CRM), including daily and weekly call schedules, track all appointmentments and activities.
Communicate, appropriately, with providers, attending physicians, other hospice staff members, and other agencies as needed to ensure excellent provider relations Maintain confidentiality of sensitive information.
Work closely with the hospice staff, Director-Clinical Operations, and all Team Directors to insure that promised services are provided Participate in hospice orientation and ongoing education programs.
Participate in committees and on special projects, as assigned Demonstrate proficiency and willingness with sign-ons and information sessions, as assigned.
Demonstrate 100% compliance with all reporting processes.
Adhere to monthly community education budgets as set by program site leadership.
Document customer service issues on the Adverse Event Form according to protocol.
Complete and maintain all HCC competencies Qualifications Hospice Care Consultant
Qualifications:
Bachelor's degree, preferred -or- 5
years' experience in hospice, home healthcare sales, or marketing, in lieu of degree Demonstrated ability to work closely with a variety of stakeholders, effective problem-solving skills, and understanding of referral patterns.
Experience with, or willing to learn, Medicare regulations and related reimbursement practices related to hospice and long-term care.
Demonstrate familiarity with policies of the agency and state(s) and federal regulations pertaining to hospice services Available for after hours and weekend referrals Recommended Skills Long Term Care Assessments Utilization Management Hospice Case Management Nursing Home Care Estimated Salary: $20 to $28 per hour based on qualifications.
