Candidate must be a RN (Registered Nurse).
The primary responsibility of the Per Diem RN Case Manager, is to assess MediCal beneficiaries for the Assisted Living Waiver Program.
The RN Case Manager will be working with seniors and persons with disabilities that require nursing facility care, but can safely live in an Assisted Living Facility (RCFE).
The position requires working from home and traveling to various medical facilities throughout Sacramento and San Joaquin County to perform assessments and other case management duties.
The Per Diem RN Case Manager maintains a comprehensive understanding of ethical principles and independence in their work, professional conduct, duty, and responsibilities.
The position is a Per Diem contract position with paid mileage.
Essential Duties and
Responsibilities:
Work with seniors and persons with disabilities that require nursing facility care Conduct face to face and telephonic interviews with MediCal beneficiaries and, as appropriate, contact with family members, legal representatives and/or other informal supports Develop individual care plans Perform initial and reassessments Work collaboratively with participating ALW providers and case managers Ensure confidentiality of medical information related to HIPAA regulations Display dedication to corporate objectives Complete work product in an accurate and timely fashion Perform other duties as assigned/necessary Desired Skills and
Experience:
Minimum Requirements:
Licensed RN with a current active California license Acute experience Preferred experience in either:
RN Case Management, Home Health, Hospice, Geriatrics Preferred residence in Sacramento County Ability to set priorities and meet timelines for performance expectations, while maintaining strong attention to detail Ability to work independently with minimal supervision, and demonstrate initiative Excellent interpersonal and communication skills Computer proficiency, including MS Office Suite, email, and internet Must own a laptop computer Must be able to travel throughout Sacramento and San Joaquin County Must have reliable transportation and the ability to pass a background check Ability to effectively elicit/provide information to and from appropriate individuals via strong communication skills; proficiency in the English language may be required Job Type:
