  • completing the initial registration paperwork and intake assessment for new members.
  • assisting in the linking and handoff at team meetings to support the development of the individual client plan in partnership with the member and the treatment team.
  • Determines a member's strengths, challenges, and family or other support system.
  • checks for treatment-related obstacles and finds a member's natural supports
  • collaborates well with the treatment team to deliver member-driven, recovery-oriented holistic evaluation and engagement services.
  • punctually completes all required annual documentation as given.
  • Supports Clinical Director to ensure effectiveness of annual charts and adherence to Medi-Cal requirements
  • provides quick response in the case of a crisis, stopping the incident and, when appropriate, alerting the people and organizations required for its resolution.
  • gives participants on-the-spot therapy that is both beneficial to them and in line with the program's principles.
  • Contact the previous provider, primary care physician, psychiatric hospital, and/or collateral resources to learn more about the assessment and to offer case management as needed during the transition.
  • Make that the agency psychiatrist is notified within 30 days of a member's discharge from a mental health facility. Make that the agency psychiatrist is contacted when a member joins services if medication is already in place.
  • During the evaluation stage, create an initial safety plan and monitor safety planning in high risk situations.
  • Complete service requests and, if necessary, connect members, their families, and other designated support systems to available community services.
  • Provide family and individual counseling in addition to group facilitation.
  • assists Family Advocate in recruiting new referrals to the program.

Qualifications
  • Valid licensure with the Board of Behavioral Services as an LCSW or LPC.
  • CA Driver's License.
  • 2 years of experience in behavioral health.
Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
